COACH LEDFORD CAMPS TERMS AND CONDTIONS

I certify that my child is medically cleared to attend this camp and I give my permission for the camp staff to act for me in the event of an emergency that requires medical attention.  I am relieving all camp staff, employees and volunteers as well as Hayes Barton Baptist/Hayes Barton United Methodist Churches from any liability in case if injury, illness, loss , damage or death that may occur at this camp.  I further understand that the terms of this agreement are legally binding and certify that I am signing this agreement, after carefully reading it, of my own free will.
